Mature cystic teratoma, also called dermoid cyst, are 95% of all germ cells derived tumors. Teratomas with a high grade differentiation are often seen but it is rare to find a teratoma with organs or complete body areas. We report in this paper a case of high grade differentiation of a mature cystic teratoma of the ovary, in which we observed a rudimentary hand. Anatomopathological findings are reported related to a 25 years old patient ovarian biopsy. Alcian Blue, Masson´s tricromic, Van Gieson for elastic fibers histochemical stains and immunohistochemistry for protein S-100 were performed. The cyst wall was internally cover by mucin epithelia without atipic characteristics or mitosis. The solid formation was made of different embryonic tissues. The rudimentary hand had cartilaginous phalanges, with a ossification zone, and bone marrow with hematopoyectic elements. The phalanges were surrounded by periostial tissue, dermis and epidermis. This case shows the histogenetic potential of this kind of tumors, in which has been demonstrated an exceptional grade of differentiation
