A four year old child with acute lymphoblastic leukemia and severe neutropenia secondary to chemotherapy, presented with periorbitarian cellulitis and proptosis of the right eye. Initially it was supposed to have a bacterial origin but he didn't improve with antibiotic treatment. As MRI detected a cerebritis focus, three surgical drainages and debridements were done and a nasal osseus biopsy confirmed the presence of Mucorales like hyphae. He was treated with amphotericin B deoxicolate, for two months entering 2,500 mg (113.6 mg/kg) associated to itraconazole, with good clinical and images resolution. The patient completed his chemotherapy and radiotherapy protocols and after 8 years of follow up he is cured of the leukemia.
