Acute renal failure has been recognized as a rare complication of non-fulminant hepatitis A infection. The association has been reported in about 30 cases, the majority proceeding from Japanese patients. The exact mechanism for acute renal failure in hepatitis A is uncertain, several possible aetiological mechanisms can be put forward: immune complex mediated glomerulonephritis, hyperbilirrubinemia, hepatorenal syndrome, acute interstitial nephritis and acute isquemic tubular necrosis. Acute tubular necrosis is the principal renal lesion in acute renal failure in hepatitis A infection. We report the first case in Chile of non-oliguric acute renal failure, a 16-year-old male, with a non-fulminant hepatitis A infection, with a very good spontaneous evolution and without need hemodialysis therapy. Because of the quick improvement of our patient and coagulation abnormalities, we did not perform a renal biopsy.
