Background: Traumatic diaphragmatic hernia is rare in children and constitutes a pathology of difficult diagnosis. It is a severity marker of trauma, because it presents high incidence of morbidity and mortality. Objective: To report a case of diaphragmatic hernia secondary to blunt chest-abdominal trauma. Case-report: An 8-years old girl who presented left diaphragmatic hernia and unstable pelvis fracture after being knocked by a car. Days after, she presented chylothorax and left kidney infarction handle with conservative treatment
