The idiopathic nephrotic syndrome (INS) in children is a benign clinical entity, because in most cases it corresponds to Minimal Change Disease, showing an intact renal function after a long period of time. Some few cases correspond to other glomerulopathies, especially Focal Segmental Glomerulo Sclerosis, with higher risk of renal failure. Although INS has a high rate of response to steroids, more than 60% of cases present numerous relapses, bringing together various complications due to the nephrotic condition itself and the prolonged use of steroids. In these patients, along with those who present steroid-resistance, other therapeutic schemes need to be applied, sometimes with important side effects. The aim of this update is to teach INS treatment based on an extended literature research; nevertheless, it is important not to forget that each patient must have its individual prescription according to risks and benefits of the therapy
