Severe traumatic brain injury (TBI) is a major cause of disability and death in the paedriatric population. A rational therapeutic approach is possible as a result of understand brain"s physiology, pathophysiological changes triggered by trauma and the use of modern methods of neurological monitoring. This article presents an overview of the current advances in the care of critically ill patients and experimental treatments. A standardized treatment protocol of severe TBI is proposed in order to unify criteria, collect comparable data and avoid or minimise neurological damage
