The tension-free vaginal tape for female urinary incontinence has become very popular since the 90's. Most of the reports show bladder lesions as the major complication of the procedure. Inspired by Delorme, we have developed a procedure placing the tape with needles via a transobturator approach as a way of avoiding the pass through the retropubic space, close to the bladder. We operated 18 patients between november 2002 and april 2003. We had no bladder injuries and after an average follow up of 6 months, all of the patients are continent. The surgical technique is shown and its indications discussed
