Background: The esophageal lung is an uncommon congenital malformation due to an alteration of the foregut development. It is characterized by pulmonary parenchyma connected to the esophagus. The clinical features of this condition include repeated pneumonia, respiratory distress and feeding problems. Objective: To report the first case of esophageal lung diagnosed and treated in our institution. Case-report: A 4 months-old male with right upper lobe recurrent pneumonia. The radiological and endoscopical studies confirmed the diagnosis of a supernumerary lung connected to the esophagus. Surgical resection was performed with excellent results and improvement of symptoms. Conclusions: The esophageal lung is a rare malformation that should be considered in the differential diagnosis of recurrent pneumonia. A complete pre-surgical evaluation improves accuracy for congenital malformations and facilitates the surgery
