Two female adolescents aged 13 and 14 years old were evaluated for exercise associated syncope. One of them complained of exercise and cold water induced urticaria and the other one of excercise related angioedema. Their physical examination, electrocardiogram and cardiac ultrasonography were normal. At treadmill excercise test the first girl had severe pruritus, maculopapular erythema and cholinergic urticaria and the second one had pharyngeal angioedema, coarseness and inspiratory stridor. Both patients recovered spontaneously after cessation of exercise. Avoidance of strenuous exercise and prophylactic oral antihistamines were recommended. Exercise is an uncommon cause of urticaria and angioedema. Epinephrine, airway protection, intravenous fluids and induction of tolerance may be needed in severe cases.
